We bill insurance as a courtesy to our patients. Patients are fully responsible for the bill.

Note that insurance plans change regularly and patients should call their insurance company for confirmation.

*All office services must be verified/authorized prior to visit. Surgery will be verified/authorized before scheduling.

**We do accept most other plans and nearly all Workman’s Compensation plans when the injury has occurred in Arizona.
