Workers' Compensation Injuries

The Sonoran Spine Center accepts Workers' Compensation plans when the injury has occurred in Arizona.

In order to schedule your workers' compensation patients with one of our physicians, please call our workers' compensation department at (602) 443-4160 and let them know it is a workers' compensation case, and have ready the following information:

1. Claim number
2. What is being approved – type of appointment/treatment
3. Billing address

Once the necessary case information is completed (Click for the referral form), you can schedule the patient's appointment or have the patient call us to schedule at his or her convenience.
